Publication Metadata only Arteriovenous Fistula Self-Care Behaviors in Patients Receiving Hemodialysis Treatment: Association With Health Literacy and Self-Care Agency(Sage Publications Ltd., 2022) Bülbül, Elif; Ayvaz, Meryem Yıldız; Yeni, Tuğba; TÜREN, SEVDA; Efil, SevdaBackground: Arteriovenous fistula-related self-care behaviors, self-care agency and health literacy are important for vascular access patency, which is vital in the continuation of hemodialysis treatment. The purpose of this study was to determine the arteriovenous fistula-related self-care behaviors of patients receiving chronic hemodialysis treatment and the relationship between these behaviors and their health literacy and self-care agency levels. Methods: In this descriptive correlational study, the data were collected from 216 chronic hemodialysis patients. Results: The rate of the patients who had good self-care behaviors levels was 83.96%. The sociodemographic variables that were significantly related to AVF-related self-care behaviors were education, employment status and age. It was found that the patients who had had AVF for a longer time had better self-care behaviors regarding their management of symptoms and findings (p < 0.05). As the self-care agency of the patients (r = 0.612, p < 0.001) and their health literacy (r = 0.421, p < 0.001) increased, their AVF-related self-care behaviors also increased. Conclusion: Age, education status, health literacy and self-care agency were identified to affect AVF-related self-care behaviors.Publication Restricted Assessment of the Correlation Between Weight Status and the Frequency of Dietician Interviews in Sleeve Gastrectomy Patients(Springer, 2021) BATAR, NAZLI; SEVDİN, SEZEN; Güçlü, Duygu; KERMEN, SEDAPurpose The aim of this study was to evaluate the relationship between the weight status of sleeve gastrectomy patients during the first two postoperative years and the frequency of dietician interviews. Materials and Methods This was a retrospective cross-sectional study, and the subjects were patients who had surgery at a special obesity clinic, Bariatriklab, in Istanbul, Turkey, between February 01, 2014, and February 01, 2018.The mean age +/- standard deviation of the participants was 38.8 +/- 11.9 years. The mean body mass index (BMI) +/- standard deviation was 44.2 +/- 6.1 kg/m(2)preoperatively. The preoperative (pre-op) and 12-, 18-, and 24-month postoperative (post-op) weights and the frequency of interviews with dieticians for 2 years after the operation were collected, and statistical analysis was performed using SPSS ver. 23.0. Results A total of 247 laparoscopic sleeve gastrectomy (LSG) patients, 161 women (65.2%) and 86 men (34.8%), were evaluated in this study. As the frequency of dietician interviews increased in the first year, the amount of excess weight loss (EWL) increased in the second year (p < 0.01). In addition, as the frequency of dietician interviews increased in the second year, the amount of EWL in the second year decreased (p < 0.01). The frequency of dietician counseling among participants who did not experience weight loss between 12 and 24 months was significantly higher than that among those who experienced weight loss (p < 0.05). Conclusion It should not be forgotten that bariatric surgery requires teamwork, and patients should be cared for with an interdisciplinary approach. Dieticians play an important role in changing nutritional habits and making them sustainable.Publication Metadata only COVID-19 Fear, Vaccination Hesitancy, and Vaccination Status in Pregnant and Breastfeeding Women in Turkey(Afr J Reprod Health, 2023) MİRAL, MUKADDES TURAN; Turgut, Nurgül; Güldür, Ayşe; Güloğlu, Zübeyde E.; Mamuk, RojjinThis study aimed to determine the level of COVID-19 fear, vaccination, and vaccination hesitancy as well as the affecting factors in pregnant and breastfeeding women who participated in an online prenatal education in Turkey. The study, which was designed as descriptive cross-sectional, was conducted online with 360 pregnant and breastfeeding women from Istanbul. Data were collected through the Participant Information Form, Fear of COVID-19 Scale and Vaccine Hesitancy Scale in Pandemics. The rate of accepting the COVID-19 vaccine is 65.6%. The Fear of COVID-19 Scale was 16.215.54, and the Vaccine Hesitancy Scale in Pandemics mean score was 29.294.54. The COVID-19 fear of the women participating in this study was moderate, the level of vaccination hesitancy was low, and two-thirds of them were vaccinated. There is a need to organize special counseling and vaccination campaigns for pregnant and lactating women.Publication Metadata only Determination of Factors Affecting Time in Therapeutic Range in Patients on Warfarin Therapy(Sage Publications Inc., 2023) TÜREN, SEVDA; Türen, SelahattinBackground The effectiveness and safety of warfarin depend on maintaining an international normalized ratio (INR) within the therapeutic range. Time in Therapeutic Range (TTR) is defined as the percentage of time a patient's INR is within the therapeutic range. Objective We sought to determine the factors affecting good TTR in patients on warfarin therapy. Methods This was a descriptive cross-sectional study conducted in a single tertiary care center. Good anticoagulation control was defined as TTR >= 65%. Results The study population consisted of 518 patients. The mean age was 57.6 +/- 12.3 (19-87) and 54.4% of the patients were female. 47.5% patients achieved good anticoagulation control (TTR >= 65%). The mean Medication Adherence Report Scale (MARS) score was significantly higher in patients with good TTR (23.5 +/- 1.9 vs. 22.8 +/- 2.1, p = .002). Only 40.2% of the patients received education on warfarin. In multivariable analyses, the duration of warfarin therapy >10 years (OR: 2.27, 95% CI: 1.34-3.84, p = .002) and MARS score (OR: 1.22, 95% CI: 1.09-1.35, p < .001) were found to be the independent predictors of the good anticoagulation control. Conclusion Duration of warfarin therapy >10 years and MARS score were the independent predictors of good anticoagulation control.Publication Restricted Epibrassinolide-Induced Autophagy Occurs in an Atg5-Independent Manner Due to Endoplasmic Stress Induction in MEF Cells(Springer, 2020) Adacan, Kaan; YERLİKAYA, PINAR OBAKAN; Çoker-Gürkan, Ajda; Kaya, Resul İsmail; Palavan-Ünsal, Narcin; ARISAN, ELİF DAMLAEpibrassinolide (EBR), a polyhydroxysteroid belongs to plant growth regulator family, brassinosteroids and has been shown to have a similar chemical structure to mammalian steroid hormones. Our findings indicated that EBR could trigger apoptosis in cancer cells via induction of endoplasmic reticulum (ER) stress, caused by protein folding disturbance in the ER. Normal cells exhibited a remarkable resistance to EBR treatment and avoid from apoptotic cell death. The unfolded protein response clears un/misfolded proteins and restore ER functions. When stress is chronic, cells tend to die due to improper cellular functions. To understand the effect of EBR in non-malign cells, mouse embryonic fibroblast (MEF) cells were investigated in detail for ER stress biomarkers, autophagy, and polyamine metabolism in this study. Evolutionary conserved autophagy mechanism is a crucial cellular process to clean damaged organelles and protein aggregates through lysosome under the control of autophagy-related genes (ATGs). Cells tend to activate autophagy to promote cell survival under stress conditions. Polyamines are polycationic molecules playing a role in the homeostasis of important cellular events such as cell survival, growth, and, proliferation. The administration of PAs has been markedly extended the lifespan of various organisms via inducing autophagy and inhibiting oxidative stress. Our data indicated that ER stress is induced following EBR treatment in MEF cells as well as MEF Atg5(-/-) cells. In addition, autophagy is activated following EBR treatment by targeting PI3K/Akt/mTOR in wildtype (wt) cells. However, EBR-induced autophagy targets ULK1 in MEF cells lacking Atg5 expression. Besides, EBR treatment depleted the PA pool in MEF cells through the alterations of metabolic enzymes. The administration of Spd with EBR further increased autophagic vacuole formation. In conclusion, EBR is an anticancer drug candidate with selective cytotoxicity for cancer cells, in addition the induction of autophagy and PA metabolism are critical for responses of normal cells against EBR.Publication Metadata only Functional Bandaging in Children with Idiopathic Toe-Walking(NLM (Medline), 2023) Tuncer, Deniz; AKALAN, NAZİF EKİN; Çalışkan, M. Mine; Temelli, Yener; Yiğit, PakizeBackground: Idiopathic toe-walking (ITW) is a persistent gait pattern with no known etiology characterized as premature heel rise or no heel contact. We investigated the effects of functional bandaging in children with ITW on heel contact during stance phase and on gait quality. Methods: Nineteen children aged 4 to 16 years with ITW and ten age-matched healthy children were included in the study. Elastic adhesive bandages were applied to children with ITW to assist with dorsiflexion. Before bandaging (T0) and immediately (T1) and 1 week (T2) after initial bandaging, the initial contact, loading response, and midstance subphases of gait were analyzed using light pressure sensors and the Edinburgh Visual Gait Score (EVGS). Ten age-matched children with typical gait participated for comparison in T0. The data were analyzed with Friedman and Wilcoxon signed rank tests for within-group comparisons and Mann-Whitney U tests for between-group comparisons. Results: In T0, for the ITW group, no heel contact was observed during stance. In T1, all of the participants achieved heel contact at initial contact and loading response and 56.8% at midstance. In T2, all of the heels continued contact at initial contact and loading response and 54.3% at midstance. The EVGS significantly improved. The Friedman test showed that there were noteworthy improvements between T0-T1 and T0-T2 in video-based observational gait analysis and EVGSs (P < .001), although no difference was found between T1-T2 in video-based observational gait analysis (P = .913) and EVGSs (P = .450). Conclusions: In children with ITW, dorsiflexion assistive functional bandaging was an effective tool to help achieve heel contact on the ground and improve walking quality for a short period after application. Further studies with longer follow-up and larger sample sizes are required to confirm the long-term therapeutic effects of this promising functional bandaging.Publication Metadata only The Most Commonly Determined Nursing Diagnoses According to NANDA-I Taxonomy and Difficulties Using the Nursing Process in COVID-19 Outbreak(Sage Publications Inc., 2022) Tosun, Hale; Tosun, Ayşe; Özkaya, Birgül Odul; GÜL, ASİYEThe study was planned to determine the most common nursing diagnoses according to NANDA International (NANDA-I) taxonomy and difficulties experienced in using of nursing process in COVID-19 outbreak. The sample of the descriptive cross-sectional study consisted of nurses cared for patients with COVID-19 (n = 114). Average age of nurses is 26.86 +/- 6.68. Commonly determined nursing diagnoses according to NANDA-I taxonomy in patients with COVID-19 were imbalanced nutrition (66.7%), impaired gas exchange (40.4%), insomnia (21.1%), acute confusion (31.6%), hopelessness (96.5%), difficulty playing caregiver (84.2%), anxiety (38.6%) willingness to strengthen religious bond (71.9%), risk for infection (64.9%), nausea (49.1%). Twenty-four-years old and younger, high school graduates, caring for intubated patients, and those who stated that they did not use nursing diagnosis had more difficulty in using nursing process (<0.05). The use of nursing diagnoses and process for patients with COVID-19 is extremely important in ensuring individual and qualified nursing care.Publication Metadata only Presence of Foodborne Pathogens in Seafood and Risk Ranking for Pathogens(Mary Ann Liebert, Inc., 2020) Dümen, Emek; EKİCİ, GÖZDE; Ergin, Sevgi; Bayrakal, Gülay MerveThis study aims at examining the contamination of coliform bacteria, Escherichia coli, Listeria monocytogenes, Vibrio vulnificus, and Vibrio cholerae, which carry extremely serious risks to the consumer health, in 700 seafood belonging to 4 different (raw sea fish, raw mussels, raw shrimp, and raw squid) categories. The total number of samples was determined as 700. When the obtained results were viewed in total, they were found to be 48.14%, 18.71%, 8.57%, and 3.42% for coliform bacteria, E. coli, L. monocytogenes, and V. vulnificus, respectively. V. cholerae, one of the factors studied, was not found. Conventional microbiological cultivation methods were used in the analysis stage as well as the real-time PCR method. This study aims at making a risk ranking modeling for consumer health based on product category and pathogens by interpreting the results of the analysis with statistical methods. According to the statistical analysis, significantly binary correlations were determined among some parameters that stimulate one another for reproducing. In the light of the obtained results of the study, it has been concluded that the studies of the most detailed examinations of the microbiological risks associated with seafood, forms of microbial pollution and microorganisms that cause deterioration in seafood and threaten consumer health and the path that their epidemiologies follow, are of primary importance to both protecting consumer health and obtaining safe and quality seafood.Publication Restricted Proinflammatory Cytokine Profile is Critical in Autocrine GH-Triggered Curcumin Resistance Engulf by Atiprimod Cotreatment in MCF-7 and MDA-MB-231 Breast Cancer Cells(Springer, 2020) Çoker-Gürkan, Ajda; Özakaltun, Buse; Akdeniz, Berre-Serra; Ergen, Berfin; YERLİKAYA, PINAR OBAKAN; Akkoç, Tunç; ARISAN, ELİF DAMLAActive growth hormone (GH) signaling triggers cellular growth and invasion-metastasis in colon, breast, and prostate cancer. Curcumin, an inhibitor of NF-kappa B pathway, is assumed to be a promising anti-carcinogenic agent. Atiprimod is also an anti-inflammatory, anti-carcinogenic agent that induces apoptotic cell death in hepatocellular carcinoma, multiple myeloma, and pituitary adenoma. We aimed to demonstrate the potential additional effect of atiprimod on curcumin-induced apoptotic cell death via cytokine expression profiles in MCF-7 and MDA-MB-231 cells with active GH signaling. The effect of curcumin and/or atiprimod on IL-2, IL-4, and IL-17A levels were measured by ELISA assay. MTT cell viability, trypan blue exclusion, and colony formation assays were performed to determine the effect of combined drug exposure on cell viability, growth, and colony formation, respectively. Alteration of the NF-kappa B signaling pathway protein expression profile was determined following curcumin and/or atiprimod exposure by RT-PCR and immunoblotting. Finally, the effect of curcumin with/without atiprimod treatment on Reactive Oxygen Species (ROS) generation and apoptotic cell death was examined by DCFH-DA and Annexin V/PI FACS flow analysis, respectively. Autocrine GH-mediated IL-6, IL-8, IL-10 expressions were downregulated by curcumin treatment. Atiprimod co-treatment increased the inhibitory effect of curcumin on cell viability, proliferation and also increased the curcumin-triggered ROS generation in each GH(+) breast cancer cells. Combined drug exposure increased apoptotic cell death through acting on IL-2, IL-4, and IL-17A secretion. Forced GH-triggered curcumin resistance might be overwhelmed by atiprimod and curcumin co-treatment via modulating NF-kappa B-mediated inflammatory cytokine expression in MCF-7 and MDA-MB-231 cells.Publication Restricted PyTHang: an Open-s-Source Wearable Sensor System for Real-Time Monitoring of Head-Torso Angle for Ambulatory Application(Taylor & Francis Ltd., 2021) GÜRKAN, GÜRAYThis article presents the realization of a low-cost wearable sensor system and its Python-based software that can measure and record relative head-torso angle, especially in sagittal plane. The system is mainly developed to track head-torso angle during walk in a clinical study. The open-hardware part of the system is composed of a pair of triaxial digital accelerometers, a microprocessor, a Bluetooth module and a rechargeable battery unit. The reception of the transmitted acceleration data, visualization, interactive sensor alignment, angle estimation and data-logging are realized by the developed open-source graphical user interface. The system is tested on a tripod for verification and on a subject for practical demonstration. Developed system can be constructed and used for ambulatory monitoring and analysis of relative head-torso angle. Open-source user interface can be downloaded and developed for further (different) algorithms and device hardware.Publication Metadata only "Sleep Quality and Depression Level in Nurses in COVID-19 Pandemic"(Sage Publications Inc., 2022) Tosun, Ayşe; Tosun, Hale; Özkaya, Birgül Ödül; Erdoğan, Zeynep; GÜL, ASİYEThe COVID-19 spread rapidly all over the world and pandemic process has negatively affected nurses. Sleep disorders and depression are among these negative effects. Managers' awareness of problems experienced by nurses and taking precautions will increase employee and patient satisfaction and provide quality patient care. The study was conducted in descriptive, cross-sectional and correlational design to explain nurses' sleep quality and depression levels and relationship between them during the COVID-19 pandemic with 142 nurses who completed Personel Information Form, Pittsburgh Sleep Quality Index (PSQI) and Beck Depression Index (BDI). Sleep quality of nurses (64.8%) was poor, 33.1% had depression, and high school education, having a chronic illness and PSQI score increased the risk of depression level of nurses. Results highlight COVID-19 pandemic affected nurses' sleep quality and depression levels. All managers should be pioneers in providing psychological support to nurses and preparing and implementing a program for prevention of insomnia and depression.Publication Restricted Structural and Spectral Analysis of Anticancer Active Cyclo(Ala-His) Dipeptide(Taylor & Francis Inc., 2020) Çelik, Sefa; YILMAZ, GÖZDE; Özel, Ayşen E.; AKYÜZ, SEVİMThe theoretically possible most stable conformation of the cyclic dipeptide, which has a significant anticancer activity, was examined by conformational analysis method and then by DFT calculations. With DFT calculations, cyclo(Ala-His) dipeptide was found to be more stable in boat form than in planar conformation. Moreover, conformations of the dimeric forms of the title molecule were investigated. The dimeric forms of the cyclo(Ala-His) dipeptide were created by combining two identical cyclo(Ala-His) monomers, in lowest energy configuration and as a result three energetically possible dimeric structures were obtained. The solid phase FTIR and Raman spectra of cyclo(Ala-His) have been recorded. The spectra were interpreted with the aid of quantum chemical calculations based on density functional theory, using B3LYP and wb97xd methods with 6-311++G(d,p) basis set, in order to elucidate structural and spectral properties of the investigated molecule. Experimental vibrational spectra are found to be in accord with the simulated vibrational spectra. The assignment of the vibrational modes was performed depending on the calculated potential energy distribution (PED).In slicomolecular docking of cyclo(Ala-His) was also carried out with DNA. The drug likeness and ADMET properties were analyzed for the prediction of pharmacokinetic profiles. The results revealed that the compound has the potential to be the leading molecule in the drug discovery process.Publication Metadata only To Operate or Not to Operate? Stepwise multivariate linear regressions were performed for each QoL domain. Setting: Urban tertiary care center Patients, participants: Pediatric patients with facial differences, and their parents. Intervention: Survey study Main Outcome Measure(s): Demographic, diagnostic, and surgical characteristics were collected. Surgical burden was calculated as the standard deviation from the mean number of surgeries per diagnostic cohort. Result: Patients (N = 168) were majority female (57.1%) and Hispanic (64.3%). Diagnoses were cleft lip and/or palate (CLP,n = 99) or other craniofacial conditions (CFC,n = 69). Average patient age was 2.3 +/- 5.6 years at first reconstructive surgery and 12.3 +/- 3.4 years at study enrollment. Patients received an average of 4.3 +/- 4.1 reconstructive surgeries. Worse Bullying was associated with higher surgical burden. Worse Peer Problems was associated with higher surgical burden, but only for children with non-CLP CFCs. Worse Family Support was associated with CFC diagnosis, female sex, and higher surgical burden. Worse Psychological Impact was associated with higher surgical burden. Worse Appearance Satisfaction was associated with younger age and with lower surgical burden. Greater Desire for Appearance Change was associated with older age, higher surgical burden, CLP diagnosis, female sex, and non-Hispanic ethnicity. Socioeconomic status did not predict QoL per patient self- or parent-proxy report. Conclusions: Higher surgical burden was associated with worse QoL outcomes in multiple domains.Publication Restricted Wearable Sensor-Based Evaluation of Psychosocial Stress in Patients With Metabolic Syndrome(Elsevier, 2020) AKBULUT, FATMA PATLAR; İkitimur, Barış; Akan, AydınThe prevalence of metabolic disorders has increased rapidly as such they become a major health issue recently. Despite the definition of genetic associations with obesity and cardiovascular diseases, they constitute only a small part of the incidence of disease. Environmental and physiological effects such as stress, behavioral and metabolic disturbances, infections, and nutritional deficiencies have now revealed as contributing factors to develop metabolic diseases. This study presents a multivariate methodology for the modeling of stress on metabolic syndrome (MES) patients. We have developed a supporting system to cope with MES patients' anxiety and stress by means of several biosignals such as ECG, GSR, body temperature, SpO(2), glucose level, and blood pressure that are measured by a wearable device. We employed a neural network model to classify emotions with HRV analysis in the detection of stressor moments. We have accurately recognized the stressful situations using physiological responses to stimuli by utilizing our proposed affective state detection algorithm. We evaluated our system with a dataset of 312 biosignal records from 30 participants and the results showed that our proposed method achieved an average accuracy of 92% and 89% in distinguishing stress level in MES and other groups respectively. Both being the focus of an MES group and others proved to be highly arousing experiences which were significantly reflected in the physiological signal. Exposure to the stress in MES and cardiovascular heart disease patients increases the chronic symptoms. An early stage of comprehensive intervention may reduce the risk of general cardiovascular events in these particular groups. In this context, the use of e-health applications such as our proposed system facilitates these processes.
